By accessing or using the Ava Health Provider Directory ("Directory") at providers.avahealth.co, you agree to be bound by these Terms of Use. If you do not agree to these terms, please do not use the Directory.

2. Description of Service

The Directory is a free, publicly accessible reference tool that displays healthcare provider information sourced from the National Plan and Provider Enumeration System (NPPES), maintained by the Centers for Medicare & Medicaid Services (CMS). The Directory is operated by Ava Health.

3. Data Source and Accuracy

All provider information displayed in the Directory is obtained from the NPPES public data dissemination, which is a public record maintained by the U.S. federal government. While we strive to keep the Directory current and accurate, we make no guarantees regarding the completeness, accuracy, or timeliness of the information displayed.

Provider credentials, specialties, and practice locations may change. Users should independently verify provider information before making healthcare decisions. To report inaccurate information, providers should update their records directly with CMS at nppes.cms.hhs.gov.

4. Not Medical Advice

The Directory is for informational purposes only and does not constitute medical advice, diagnosis, treatment recommendations, or an endorsement of any healthcare provider. The inclusion of a provider in the Directory does not imply a recommendation by Ava Health. Always consult with a qualified healthcare professional for medical advice.

5. No Provider-Patient Relationship

Use of the Directory does not create a provider-patient relationship between you and any listed healthcare provider, nor between you and Ava Health. Ava Health is a healthcare staffing company and does not provide medical services through this Directory.

6. Acceptable Use

You agree not to:

  • Use the Directory for any unlawful purpose
  • Scrape, harvest, or systematically download data from the Directory in bulk
  • Use the Directory to harass, stalk, or contact providers for unsolicited commercial purposes
  • Attempt to gain unauthorized access to our systems or data
  • Interfere with or disrupt the operation of the Directory
  • Reproduce, distribute, or create derivative works from the Directory without permission

7. Intellectual Property

The design, layout, and original content of the Directory (excluding provider data sourced from NPPES) are the property of Ava Health and are protected by applicable intellectual property laws. The underlying provider data is sourced from publicly available government records.

8. Disclaimer of Warranties

THE DIRECTORY IS PROVIDED "AS IS" AND "AS AVAILABLE" WITHOUT WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, ACCURACY, OR NON-INFRINGEMENT. AVA HEALTH DOES NOT WARRANT THAT THE DIRECTORY WILL BE UNINTERRUPTED, ERROR-FREE, OR FREE OF HARMFUL COMPONENTS.

9. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, AVA HEALTH AND ITS OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE DIRECTORY, INCLUDING BUT NOT LIMITED TO DAMAGES RESULTING FROM RELIANCE ON PROVIDER INFORMATION DISPLAYED IN THE DIRECTORY.

The Directory may contain links to third-party websites, including avahealth.co. We are not responsible for the content, privacy practices, or terms of use of any linked sites.

11. Modifications

We reserve the right to modify these Terms of Use at any time. Changes will be posted on this page with an updated date. Continued use of the Directory following any changes constitutes acceptance of the revised terms.

12. Governing Law

These Terms of Use shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions.
