Provider listing removal

Ava Health has discontinued its public provider directory. The directory previously indexed publicly available information from the National Plan and Provider Enumeration System (NPPES NPI Registry), a public database maintained by the Centers for Medicare & Medicaid Services (CMS). We never displayed email addresses, personal phone numbers, or any data outside of NPPES.

Individual provider listing pages have been removed and now return a 404. If you previously found your information listed here and still have a concern, including a search engine showing a cached copy, email us and we'll help.

Ava Health is a healthcare staffing and recruiting firm, not a provider-review or referral service. For current provider licensing and standing, check with the relevant state licensing board or the NPPES NPI Registry directly.
