What Is a Surgical Technologist?

Surgical technologists (also called scrub techs or surgical techs) are allied health professionals who prepare the sterile field for surgical procedures, maintain the integrity of the sterile environment, pass instruments and supplies to the surgeon and first assistant during procedures, and assist in wound closure and specimen handling. They work under the supervision of the surgeon and circulating RN in the operating room.

The surgical technologist is responsible for having every instrument, sponge, suture, and implant ready before the surgeon asks for it. Anticipation — knowing what instrument comes next in a procedure — is the core skill that separates a competent surgical tech from an excellent one. This mastery comes through repetition and specialty-specific training.

Cardiac and robotic surgery techs command the highest premiums due to the complexity and specialized equipment. Surgical technologists who become first assistants (CST/CFA) can earn $80,000–$110,000.

Education and Certification

Surgical Technologist Education Programs

The standard educational path is a certificate (9–12 months) or associate's degree (2 years) from an accredited surgical technology program. CAAHEP (Commission on Accreditation of Allied Health Education Programs) is the accrediting body for surgical technology programs. Florida has multiple accredited programs: Valencia College, Hillsborough Community College, Fortis College, and others. The curriculum covers surgical anatomy, sterile technique, anesthesia and patient care, pharmacology for the OR, and the major surgical specialties with lab and clinical experience.

CST — Certified Surgical Technologist

Awarded by the National Board of Surgical Technology and Surgical Assisting (NBSTSA). The CST is the primary professional credential for surgical technologists.

Requirements:

  • Graduation from a CAAHEP-accredited surgical technology program (or completion of a military surgical technology program), OR
  • Employment as a surgical technologist since January 1, 2009 and 7 years of experience
  • Passing the CST examination (200 questions covering perioperative care, basic science, surgical procedures, and professional standards)
  • Renewal: every 4 years (30 continuing education credits or re-examination)

The Surgical Tech's Role in the OR: Scrub vs. Circulator

The OR team has two primary nursing/tech roles:

  • Scrub technologist: Gowned and gloved, within the sterile field. Prepares and organizes the sterile instrument setup, passes instruments to the surgeon, performs count procedures (sponges, sharps, instruments), and maintains sterile technique throughout. This is the surgical tech's primary role
  • Circulator (RN): Non-sterile, moves freely through the room. Documents, retrieves supplies, communicates with team, manages the count in partnership with the scrub, administers medications to the sterile field, and manages patient safety

A surgical technologist may sometimes perform in the circulator role (depending on state law and facility policy), but the primary function is scrubbing. The circulator role is typically RN-exclusive in most states including Florida.

Surgical Specialties: Where to Focus

Cardiac Surgery

The highest-pay, highest-demand specialty for surgical technologists. Cardiac techs (also called perfusionists' assistants or cardiac scrubs) assist with open-heart procedures: CABG, valve replacements, TAVR support procedures. Requires specialized training in cardiac anatomy, vascular anastomosis techniques, cardiopulmonary bypass equipment familiarity, and implant management. Most hospitals with cardiac programs provide specialty training; the learning curve is 12–18 months.

Robotic Surgery

da Vinci robotic surgery is now standard at most hospital systems for prostatectomy, hysterectomy, bariatric surgery, and colorectal procedures. Robotic tech training is provided by Intuitive Surgical (the da Vinci manufacturer) and supplemented by hospital orientation. Robotic-trained scrub techs are in high demand as hospitals expand robotic programs.

Orthopedic Surgery

High volume, physically demanding (heavy implant trays, power tools). Ortho techs must know total hip/knee arthroplasty, spine surgery instrumentation, and arthroscopy equipment. Strong demand in Florida given the active and aging surgical population.

Advancing to Surgical First Assistant (CSFA)

The Certified Surgical First Assistant (CSFA) credential represents a significant career advancement. First assistants actively participate in surgical procedures — providing retraction, hemostasis, suturing, and wound closure — under the surgeon's supervision. Requirements for CSFA:

  • Active CST credential
  • 5 years of surgical technology experience as a scrub tech
  • Completion of a CAAHEP-accredited surgical first assistant education program (typically 1 year, often online with clinical component)
  • Passing the CSFA examination

First assistants earn significantly more than surgical technologists ($75,000–$110,000+) and have expanded procedure involvement.

Travel Surgical Technologist

Travel surgical technologists are in high demand, particularly those with specialty training (cardiac, robotic, neurosurgery). Travel contracts for CST-credentialed OR techs offer $1,800–$2,800/week all-in, with most placements in acute care hospitals experiencing staff vacancies. Florida travel contracts are particularly active year-round due to the state's large surgical volume.
