What Is a COTA / OTA?

An Occupational Therapy Assistant (OTA) — when certified, called a COTA (Certified Occupational Therapy Assistant) — implements occupational therapy treatment plans developed by a licensed occupational therapist (OT). OTAs work directly with patients on daily living skills, fine motor activities, cognitive rehabilitation, adaptive equipment training, and vocational preparation. The work spans physical recovery (hand therapy, stroke rehabilitation) and functional independence (bathing, dressing, meal preparation, community mobility).

Like the PT/PTA distinction, the OT/OTA relationship is supervisory — OTAs work under OT oversight and cannot independently evaluate patients or create plans of care. But within that framework, OTAs provide the majority of hands-on direct treatment time with patients across many settings.

COTAs in SNF and home health settings typically earn more than in schools or outpatient clinics due to productivity incentives and higher reimbursement rates in post-acute care settings. Becoming a COTA: Education and Certification

Associate of Applied Science in Occupational Therapy Assisting

The entry-level education requirement is an associate's degree from a ACOTE-accredited OTA program (Accreditation Council for Occupational Therapy Education). These 2-year programs combine academic coursework (OT theory, anatomy, mental health, pediatrics, geriatrics) with 16 weeks of full-time Level II fieldwork (clinical rotations). Florida accredited OTA programs include: Palm Beach State College, Santa Fe College, Keiser University, Daytona State College, and others.

NBCOT Certification Exam (COTA)

After graduation from an ACOTE-accredited program, graduates must pass the NBCOT (National Board for Certification in Occupational Therapy) certification examination for OTAs. The exam tests occupational therapy theory, evaluation support, intervention skills, and professional standards. Florida requires NBCOT COTA certification for state licensure.

After passing the NBCOT exam, graduates apply to the Florida Department of Health for OTA licensure. Processing typically takes 4–8 weeks.

The bridge typically adds 2–3 years of graduate study. The decision depends on career goals: COTAs who want full independent evaluation authority and greater scope should pursue the bridge; those who are satisfied with direct treatment practice often choose to advance through COTA seniority, specialization, and leadership instead.
