OR RN Salary in 2026: What Operating Room Nurses Earn (Perioperative RN Pay Guide)
OR RN Salary in 2026
Operating room nurses — also called perioperative nurses or surgical nurses — are among the most specialized RNs in hospital settings. OR RNs work in high-acuity, sterile surgical environments alongside surgeons and anesthesiologists, managing patient care before, during, and after surgical procedures.
OR nursing requires specialized training and is not a new-grad specialty at most facilities. The investment pays off: OR RNs are consistently among the higher-paid hospital-based nurses in 2026, with strong demand across surgical specialties.

What Does an OR RN Do?
OR RNs typically serve one of two roles in the surgical suite:
Circulating RN: Manages the overall care of the patient during surgery. Responsibilities include: pre-op patient assessment, positioning, Foley catheter placement, coordinating with surgical team, maintaining sterile field integrity, managing the OR suite during the case, and documenting the surgical procedure.
Scrub RN (or Scrub Tech): Works within the sterile field alongside the surgeon. Passes instruments, counts sponges and sharps, anticipates surgical needs, manages the sterile back table. Note: some facilities use Surgical Technologists (CST) in the scrub role rather than RNs.
Most OR RN positions cross-train both roles. OR shifts are typically 8–12 hours with call requirements for scheduled and emergency cases after hours.
OR RN Surgical Specialties and Pay Impact
OR RNs who specialize in high-acuity surgical areas typically command higher compensation:
- Cardiac/CV surgery: Open heart, CABG, valve repair — highest complexity; $5,000–$12,000 above general OR
- Neurosurgery: Brain and spine cases; long procedures; premium pay at many facilities
- Orthopedics: Joint replacement, complex trauma; high volume; steady demand
- Transplant: Liver, kidney, heart — 24/7 availability required; typically $5,000–$10,000 premium
- Robotics (da Vinci): Robotic-assisted surgery; training investment from facility; competitive pay to retain certified staff
CNOR Certification: Worth It?
The Certified Perioperative Nurse (CNOR) credential from the Competency and Credentialing Institute is the gold-standard OR nursing certification. Requirements: 2 years of perioperative experience, 2,400 hours in operative and invasive care practice in the last 3 years, and passing the CNOR exam.
CNOR holders typically earn $3,000–$8,000 more annually than non-certified peers. Most hospital systems reimburse exam fees and some offer one-time certification bonuses. For experienced OR RNs, CNOR provides both credentialing recognition and salary leverage.
OR RN Demand in 2026
Elective surgery volumes — which dropped sharply during COVID-19 and recovered inconsistently — have now stabilized above pre-pandemic levels at most hospital systems. Combined with the ongoing nursing shortage and the specialized nature of OR nursing (not a quick-train specialty), OR RN demand is elevated across the country.
Florida health systems are actively recruiting OR RNs, particularly for high-volume surgical programs in growing markets. Southwest Florida in particular has seen significant surgical volume growth driven by population increases and an older patient demographic that drives higher per-capita surgical rates. Hospital systems in the Naples-Fort Myers corridor are recruiting for OR RN positions across multiple surgical specialties, with relocation assistance and competitive compensation packages.
If you're an experienced OR RN open to relocation, Southwest Florida is one of the more active hiring markets in 2026 — with the Florida no-state-income-tax advantage compounding the base compensation differential.
