Aesthetic Nurse Salary in 2026

Aesthetic nursing — also called medical aesthetics nursing or cosmetic nursing — covers injectors (Botox, fillers), laser technicians, and skincare nurses working in medical spas, dermatology offices, and plastic surgery practices. It's a fast-growing specialty with highly variable compensation depending on whether the nurse is on salary, commission, or a hybrid structure.

Salary by Setting and Structure

SettingStructureAvg Annual Compensation
Medical spa (chain)Hourly + commission$68,000–$100,000
Medical spa (independent)Salary or commission$65,000–$90,000
Dermatology officeSalary$72,000–$95,000
Plastic surgery practiceSalary$80,000–$105,000
Own practice / suite rentalRevenue-based (after expenses)$100,000–$200,000+
NP in aesthetics (independent)Hybrid or own practice$120,000–$180,000

The Commission Model: Where High Earners Are Made

The most significant pay variable in aesthetic nursing is the commission structure. Nurses who work as injectors in high-volume med spas — particularly in major metro markets (Miami, Dallas, Phoenix, NYC, LA) — often earn base salaries of $50,000–$65,000 plus 15–25% commission on injectable services. A skilled injector with a loyal book of clients who averages $25,000–$40,000 in monthly service revenue can gross $130,000–$180,000+ in total compensation.

The trade-off: commission income is variable. New injectors building a clientele may earn less in the first 12–18 months than an equivalent staff RN in clinical nursing. The ceiling is real, but so is the ramp-up period.

Certifications and Training for Aesthetic RNs

Aesthetic nursing is one of the few nursing specialties where formal specialty certification is less established than hands-on training. Key credentials and programs:

  • CANS (Certified Aesthetic Nurse Specialist): The primary national credential, issued by ISPAN. Requires 2,000 hours in aesthetic nursing and an exam. Premium: recognized by practices as a quality marker; increasingly listed in job postings as preferred.
  • Allergan Aesthetics Training: Industry-sponsored (not a license, but widely respected). Botox and filler injection training through Allergan's Medical Institute. Many new injectors start here.
  • Galderma Training: Similar structured training for Restylane and Sculptra.
  • Laser certification: State-specific requirements vary widely; some states require specific training hours for laser and intense pulsed light (IPL) procedures. Check your state's board of nursing scope-of-practice guidance.

State Scope-of-Practice: The Critical Variable

Aesthetic nursing scope varies dramatically by state. Some states allow RNs to inject autonomously under a collaborating physician agreement (physician doesn't need to be on-site). Others restrict injectable procedures to physicians-only or require physician supervision. NPs have broader autonomous practice in compact and full-practice-authority states. Before entering aesthetic nursing, verify your state's current RN scope for injectable procedures — it directly determines which positions you qualify for and whether you can own a practice.

States with NP full practice authority (Arizona, Colorado, Washington, Oregon, and 26 others as of 2026) allow NPs to open independent aesthetic practices without a physician agreement — this is where the highest-earning independent injector opportunities exist.

Med Spa Ownership: The High-Ceiling Path

NPs and (in some states) RNs who open their own aesthetic suite or medical spa can earn $150,000–$250,000+ once established. The capital investment is meaningful ($30,000–$150,000 for equipment, buildout, and startup supplies), but the unit economics of aesthetic services are strong: Botox has ~65% margins, fillers ~50%, and laser services ~70%+ after consumables. Overhead in a suite rental model (renting room space in a collaborative practice or building) is lower than a standalone clinic. Many independent injectors are generating $600,000–$1.2M+ in annual gross revenue in strong markets, netting $180,000–$300,000+ after expenses.

Career Path to Aesthetic Nursing

Most aesthetic nursing positions require 2–5 years of RN experience before entry into the specialty. Preferred background:

  • ER or ICU (anatomy knowledge, fine motor skills, high-stress management)
  • Dermatology or plastic surgery RN (direct aesthetic exposure)
  • Clinic or outpatient RN (pace similar to med spa work)

New graduates rarely enter aesthetics directly. The specialty attracts experienced nurses who want better scheduling (usually M–F with no nights or weekends), a different patient population (elective vs. acute care), and significantly higher income potential than hospital staff positions.
