Occupational therapists (OTs) help patients develop, recover, or maintain the skills needed for daily living and working — particularly after illness, injury, surgery, or developmental disorders. OTs work across a wider variety of settings than most allied health professions: hospitals, schools, outpatient clinics, home health agencies, skilled nursing facilities, mental health centers, and early intervention programs. Setting significantly affects pay.

COTA vs OTR: How Credentials Work

Occupational therapy has two credential levels:

  • OTR/L (Occupational Therapist Registered/Licensed): Master's degree (MOT) or doctorate (OTD) from ACOTE-accredited program + NBCOT examination + state licensure. This is the full OT credential. All salary data in this article refers to OTR/L positions.
  • COTA (Certified Occupational Therapy Assistant): Associate's degree from ACOTE-accredited program + NBCOT exam. Works under OTR/L supervision. Average salary: $50,000–$65,000. Less autonomy, lower pay ceiling than OTR/L.

The OTD (Doctoral) has become the AOTA-preferred entry-level degree since 2025, though the MOT remains a valid entry-level credential. Entry-level salary between MOT and OTD graduates is essentially equivalent — practice experience drives pay more than degree level post-licensure.

OT Specialty Certifications

  • CHT (Certified Hand Therapist): Administered by HTCC; requires 3 years of hand therapy experience and 4,000 hours. Highest-paying OT specialty certification; hand therapy OTs earn $85,000–$115,000. Jointly available to OTs and PTs.
  • BCPR (Board Certified in Physical Rehabilitation): AOTA advanced certification; neurorehabilitation and stroke OT specialty
  • BCMH (Board Certified in Mental Health): AOTA specialty for behavioral health OTs
  • SI (Sensory Integration): University of Southern California Sensory Integration Certification; valued in pediatric OT

OT Job Market in 2026

OT demand is driven by aging population rehabilitation needs, expansion of home health services, and growing pediatric demand for developmental OT. The BLS projects 12% OT employment growth through 2033 — faster than average for all occupations. Home health OT is the fastest-growing setting, as Medicare's shift toward home-based care creates more per-patient OT utilization outside of SNFs.

Florida is an active OT market. Hospital systems, home health agencies, and SNF chains throughout the state recruit OTs for acute care, post-acute, and outpatient positions. The older demographic concentration in markets like Southwest Florida drives high per-capita demand for OT services in stroke rehabilitation, joint replacement recovery, and aging-in-place support programs. Travel OTs are also actively recruited for Florida assignments across multiple setting types.
