In 2026, L&D RN salaries range from $74,000-$138,000 annually, with meaningful premiums for high-risk OB units, MFM (maternal-fetal medicine) settings, and RNC-OB certification holders.

  • Level III / IV Perinatal Center: +$4,000-$10,000 above community, highest-risk deliveries including extreme preterm (<28 weeks), critical maternal illness; strong academic nursing pathway
  • Birth Center (Freestanding): May be slightly below hospital L&D, lower acuity, no epidural management, midwife-led model; valued by nurses who prefer physiologic birth focus

RNC-OB Certification Premium

The RNC-OB (Registered Nurse Certified in Inpatient Obstetric Nursing) from NCC adds $6,000-$14,000 annually at major health systems.
