Telehealth Psychiatry Compensation 2026: State-by-State Guide

Telepsychiatry is the highest-leverage specialty for remote work in 2026. The combination of (a) chronic shortage of psychiatrists in non-coastal states, (b) full coverage parity laws in 38 states, and (c) eight years of mature reimbursement infrastructure means a board-certified psychiatrist with 4–6 state licenses can build a $400K+/year practice without leaving home.

This is the 2026 comp guide — what to charge, where the demand is, and how to structure for tax efficiency.

Comp by State Tier (2026)

Tier 1 — Highest pay ($300K–$425K base for full-time)

  • California, New York, New Jersey, Massachusetts: high-cost markets pay top dollar but supply is competitive — most jobs are W-2 with 35–50 patient hours/week.
  • Alaska, Hawaii, Vermont, Wyoming, North Dakota: structural shortage premiums. State telepsych signing bonuses + loan repayment can push package to $475K.

Tier 2 — Mid-range ($245K–$320K)

  • Texas, Florida, Georgia, Tennessee, North Carolina, South Carolina, Virginia, Pennsylvania, Ohio
  • Mix of group practices, hospital systems, and tech-enabled platforms (Talkspace, Brightside, etc.)
  • 1099 contracts often available at $185–$220/hr (full-time equivalent)

Tier 3 — Lower pay but high-volume opportunity ($200K–$260K)

  • Mid-South + lower-population states
  • Most opportunities are 1099 platform-based
  • Volume is plentiful — saturated demand from primary-care referrals

License Stacking Strategy

The economic logic of telepsych is simple: each additional state license unlocks a new market. The math:

  • Single state, full-time W-2: capped at $300–325K typical
  • 4 state licenses, mixed W-2 + platform 1099: $400–475K achievable
  • 8+ state licenses, full 1099 platform stack: $500K+ achievable but operationally heavy

The IMLC (Interstate Medical Licensure Compact)

40 states are members. If you have a license in any compact state and meet eligibility (board certification + clean record), you can apply for additional state licenses through IMLC for ~$700/state and 6–8 weeks turnaround instead of 4–9 months.

License Stacking Order (most economically valuable, in order)

  1. Your home state (free, already have it)
  2. Neighboring high-pop states (TX, FL, NY for east-coast based; CA, AZ for west-coast based)
  3. States with no income tax + decent demand (TX, FL, NV, WA, TN)
  4. Rural shortage states with big patient pools (KY, WV, MS, AL, NM)
  5. Tier 1 premium states (NJ, MA) only after the above are running

Tax Treatment: W-2 vs 1099 Telepsych

Most established telepsychiatrists end up running a hybrid:

  • One W-2 anchor employer (state or hospital system) — provides health insurance, 401(k) match, malpractice
  • 2–4 1099 platforms supplementing — fills out hours + adds Solo 401(k) headroom

Why the hybrid wins

  • Solo 401(k) max contribution = $69,000 in 2026 (employee $23,500 + employer 25% of net SE earnings). Most W-2 plans cap combined contributions much lower.
  • Schedule C deductions: home office, computer, license fees, malpractice, internet, professional dues — typically $15–25K/year of deduction headroom for a 1099 supplement.
  • S-corp election at >$300K net 1099 income: saves 2.9% Medicare tax on the portion taken as distributions instead of salary. ~$8–14K/year.

Realistic Year-One Plan

For a board-certified psychiatrist transitioning from in-person to telehealth:

  • Months 1–3: Get IMLC enrolled. Apply for 2 high-priority state licenses (your home + one Tier 1).
  • Months 3–4: Sign one anchor W-2 (15–25 hours/week) at a state psych hospital, university clinic, or large group with strong telepsych infrastructure.
  • Months 4–6: Add one 1099 platform (Brightside or Mindful Care typically have fastest credentialing). Aim for 10–15 hours/week.
  • Months 6–9: Add 2 more state licenses via IMLC. Add a second 1099 platform.
  • Months 9–12: Optimize hours. Drop the worst-paying platform. Consider S-corp election if 1099 income exceeds $250K.

Realistic year-1 income for a moderately motivated transition: $340–410K total comp with $69K Solo 401(k) headroom + Schedule C deductions.

Red Flags to Avoid

  • Platforms requiring you to prescribe controlled substances heavily — DEA enforcement is intensifying in 2026. Cerebral and Done both faced enforcement actions; avoid platforms where ADHD/Adderall is >30% of your panel.
  • Async-heavy platforms with no synchronous component — most state boards now require live video for new patient evals. Async-only models are increasingly non-compliant.
  • Platforms that won't cover state license fees — premium platforms reimburse. Bottom-tier platforms expect you to eat them.
  • Non-compete clauses preventing you from joining other platforms — most are 6 months / 50 mile radius (largely meaningless for telepsych) but some try for "all telepsychiatry platforms in your licensed states." Don't sign that.
