What Is Per Diem Nursing?

Per diem (Latin: "per day") nurses are employed on an as-needed basis by a hospital, health system, or staffing agency. They are called in to cover shifts when the unit is short-staffed — whether due to a census surge, call-outs, or vacation coverage. Per diem RNs set their availability in advance and typically have no guaranteed hours, but they also have no requirement to accept every shift offered.

Per diem nursing is distinct from travel nursing (which involves 8–13 week fixed contracts) and float pool nursing (which is typically a permanent employee role within one health system). It occupies the space between: maximum schedule flexibility, premium pay rates, but no contractual stability.

Per Diem Nurse Pay in 2026

Per diem pay is significantly higher than staff nurse base pay, compensating for the lack of benefits and schedule guarantees. Typical premiums:

SpecialtyStaff RN (FL hourly)Per Diem PremiumPer Diem Rate
Med-Surg$28–$36/hr25–40%$36–$50/hr
ICU / Critical Care$34–$44/hr25–40%$42–$62/hr
ER$33–$43/hr25–40%$41–$60/hr
OR / Surgical$34–$48/hr20–35%$41–$65/hr
L&D$33–$46/hr20–35%$40–$62/hr
Cath Lab$36–$52/hr20–35%$43–$70/hr

Rates vary widely by hospital system, region, and specialty. Hospital-direct per diem rates are often lower than agency-arranged per diem rates because the agency charges a markup to the facility.

Who Per Diem Nursing Is Best For

Per diem is a strong fit for:

  • Nurses with primary caregiving responsibilities (young children, aging parents) who need exact schedule control
  • Working students (NP or CRNA programs) who need to balance clinical shifts with academic loads
  • Staff nurses supplementing income — many full-time employees also hold per diem positions at competing facilities, effectively working 60+ hours across two systems
  • Semi-retired nurses who want to stay clinically active without a full schedule commitment
  • New-city arrivals who want to evaluate multiple health systems before committing to a permanent role

Per diem is a poor fit for nurses who need income predictability, cannot self-fund health insurance, or who have thin emergency reserves (slow weeks are real).

How to Get Per Diem Positions in Florida

Hospital Direct Per Diem

Most health systems have a per diem or PRN (pro re nata — "as needed") pool. Apply through the hospital career portal, specifically filtering for "PRN" or "Per Diem" positions. Requirements vary but most systems want:

  • Minimum 1–2 years of experience in the relevant specialty
  • Commitment to a minimum number of shifts per month (often 2–4, including 1 weekend and 1 holiday shift per quarter)
  • Current BLS, ACLS, and specialty certifications

Staffing Agency Per Diem

Agencies like AMN Healthcare, Aya Healthcare, and local regional agencies maintain pools of per diem nurses they place across multiple facilities. Benefits: higher rates than hospital-direct, more placements, faster onboarding. Downside: less control over which specific units you work in.

Float Pool Positions

A hospital's internal float pool is technically a per diem-adjacent model — you're a permanent employee assigned across multiple units based on census. Float pool nurses typically earn a differential of 10–20% above unit-based staff, plus full benefits.

Per Diem Taxes: What You Need to Know

Per diem nurses are W-2 employees — per diem refers to scheduling, not payment structure. All wages are fully taxable, and you receive no non-taxable stipends (unlike travel nurses who maintain a tax home). Make sure agencies are withholding state income tax in the states where you are working if you cross state lines.

If you work for multiple agencies as a per diem nurse, each will issue a separate W-2. Your total income is the sum of all. Some nurses forget agency #3's W-2 arrives in February — wait for all before filing.

Protecting Income Stability on Per Diem

Per diem nurses should:

  • Register with 2–3 facilities or agencies rather than relying on one
  • Maintain a cash reserve equal to 3 months of living expenses (slow seasons are real)
  • Purchase individual health insurance if not covered elsewhere — Florida marketplace plans (ACA) vary widely in premium and coverage; compare before assuming cost
  • Track hours carefully — you may qualify for marketplace subsidy in lean months (income-based eligibility)

Southwest Florida Per Diem Market

Lee Health and Naples-area hospital systems run active per diem pools year-round, with demand peaking October–April (snowbird season). Summer months are slower but still active due to emergency and critical care census. Many SW Florida per diem nurses supplement with travel contracts in northern markets during summer when local census dips.
