Pennsylvania Physician License 2026: PA State Board Process, IMLC Path, Timeline & Common Delays

Pennsylvania is one of the most physician-rich states in the country — Pittsburgh, Philadelphia, Hershey, and Penn Medicine all draw nationally for their academic medical centers. The PA State Board of Medicine is reasonably efficient, and Pennsylvania is a 2024 IMLC member which dramatically reduces timelines for compact-eligible physicians.

IMLC eligibility

To use the IMLC pathway, you must:

  • Hold an active medical license in an IMLC member state designated as your State of Principal License (SPL)
  • Have graduated from an LCME or AOA accredited US/Canadian medical school
  • Have completed ACGME or AOA accredited residency
  • Hold board certification or be enrolled and meet eligibility
  • No history of disciplinary action, criminal conviction, or current investigation

Standard PA State Board application

  1. Create account at pals.pa.gov (Pennsylvania Licensing System)
  2. Submit application + $405 fee
  3. Trigger FCVS profile transmission (recommended) or submit credentials directly
  4. Coordinate medical school + residency + state license verifications
  5. NPDB self-query (within 6 months)
  6. Background check (state + FBI fingerprint)
  7. Wait for PA State Board review

Required documents

  • Medical school diploma + transcripts (FCVS or direct)
  • Verification of postgraduate training (each residency / fellowship)
  • USMLE / COMLEX exam history
  • ECFMG certificate (IMGs only)
  • License verification from every state ever held
  • NPDB self-query
  • Background check (state + FBI fingerprint)
  • Malpractice insurer history (current + prior 10 years)

Penn Medicine, UPMC, Jefferson — academic credentialing layer

Pennsylvania's major academic systems handle credentialing internally on top of state licensure:

  • Penn Medicine: Hospital-employed credentialing 60–90 days post-license
  • UPMC: Internal credentialing typically parallels licensing
  • Jefferson Health: Strong support for IMLC candidates
  • Geisinger Health: Concentrated in central PA — strong recruiting infrastructure

If your destination is one of these systems, the credentialing office often handles FCVS coordination on your behalf, significantly accelerating the timeline.

Renewal

  • Biennial renewal (every 2 years, December 31)

  • 100 hours CME per renewal cycle
  • Mandatory: 12 hours patient safety / risk management, 4 hours opioid education, 2 hours child abuse recognition
