How to Become a Licensed Physical Therapist Assistant in Ohio

Ohio's diverse healthcare economy — spanning major academic medical centers in Columbus and Cleveland, robust suburban outpatient markets, and a large rural SNF sector — creates consistent demand for licensed Physical Therapist Assistants. The state has one of the larger concentrations of PTA employers in the Midwest, and its PT Compact membership allows Ohio-based PTAs to extend their practice across dozens of additional states. This guide covers every step to earn your Ohio PTA license in 2026.

Step 1: Complete a CAPTE-Accredited PTA Program

Ohio PTA licensure requires graduation from a Physical Therapist Assistant program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE). CAPTE accreditation ensures the program meets national standards for curriculum, clinical education, faculty credentials, and outcomes. Ohio is home to multiple CAPTE-accredited PTA programs at community colleges and universities across the state; confirm current accreditation status at the CAPTE directory before enrolling.

Associate degree PTA programs typically run 18–24 months and include general education prerequisites plus professional coursework in anatomy, therapeutic exercise, electrophysical agents, and clinical decision-making. A minimum of 16 weeks of supervised clinical rotations (clinical affiliations) is required. Upon completing the program, obtain official transcripts and a program completion letter — both are required for the Ohio license application.

Step 2: Pass the NPTE-PTA Exam

All PTA candidates in Ohio must pass the National Physical Therapy Examination for PTAs (NPTE-PTA), administered by the Federation of State Boards of Physical Therapy (FSBPT). The exam is computer-based and consists of 200 scored items covering the major practice domains of physical therapy. A scaled score of 600 or higher is required to pass.

To sit for the exam, you must first submit an eligibility application to the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. The Board will verify your educational credentials before issuing FSBPT eligibility. Prometric testing centers across Ohio (Columbus, Cleveland, Cincinnati, Dayton, Toledo, and more) administer the exam. Candidates receive up to three attempts per year with a six-attempt lifetime cap; plan at least 6–10 weeks of focused preparation.

Step 3: Apply for Your Ohio PTA License

Ohio PTA licensure is regulated by the Ohio Occupational Therapy, Physical Therapy, and Athletic Trainers Board. After passing the NPTE-PTA, submit your complete application package:

  • State application form (available on the Ohio OT/PT/AT Board's eLicense Ohio portal)
  • Official CAPTE-program transcripts
  • FSBPT exam score verification (sent directly from FSBPT)
  • Background check and fingerprinting authorization
  • Licensure fee (approximately $50–$75 in 2026; verify current fees on the Board portal)

Processing typically takes two to four weeks for complete applications. Ohio PTA licenses renew biennially; first-renewal CE requirements are prorated based on the license issue date. Licenses issued through eLicense Ohio include a digital verification number that employers can check in real time.

PT Compact Membership

Ohio is a member of the Physical Therapy Compact. Ohio-licensed PTAs whose primary state of residence is Ohio can apply for Compact Privilege to practice in any other member state without holding a separate state license. This is particularly valuable for Ohio PTAs working with national therapy staffing firms, travel assignments, or cross-border outpatient settings near the Indiana, Kentucky, or Pennsylvania borders.

Compact Privilege requires an active, unencumbered Ohio PTA license. Applications and privilege fees are managed through the PT Compact Commission's online portal. Each privilege is state-specific and must be renewed in alignment with your Ohio license renewal cycle.

Continuing Education Requirements

Ohio PTAs must complete 20 continuing education hours during each two-year renewal cycle. The Ohio OT/PT/AT Board accepts a broad range of CE modalities:

  • Approved live courses, workshops, and conferences
  • Online/self-study CE modules from recognized providers
  • Relevant academic coursework
  • Publication of peer-reviewed research in physical therapy

Ohio does not mandate a specific mandatory-topic component for all PTAs (unlike some states with jurisprudence or abuse-recognition mandates), but practitioners should confirm any specialty-certification or employer-required CE alongside the Board's 20-hour minimum. Retain CE documentation for at least two years post-renewal in case of audit. Popular Ohio PTA CE providers include MedBridge, APTA Learning Center, and the Ohio Physical Therapy Association (OPTA).

Ohio PTA Salary Ranges

Ohio offers competitive PTA salaries relative to Midwest averages, with meaningful variation by market and setting:

  • Entry-level (0–2 years): $46,000–$54,000 per year
  • Mid-career (3–7 years): $54,000–$63,000 per year
  • Experienced (8+ years or specialty certified): $63,000–$72,000 per year
  • Travel / contract PTA: $1,500–$2,100 per week (gross)

Columbus, Cleveland, and Cincinnati offer the highest base salaries in Ohio, driven by large health system hiring programs. SNF and long-term care settings frequently offer productivity incentives on top of base pay. Rural Ohio settings may offer lower base rates but often include retention bonuses and more scheduling flexibility.
