North Carolina CRNA License 2026: APRN Requirements, Practice Model, Salary

North Carolina CRNAs are licensed as APRNs by the NC Board of Nursing. The state operates under a collaborative/supervisory practice model — CRNAs practice under a practice arrangement with a physician, the specifics of which depend on the employment setting. Major academic health systems in Charlotte, Raleigh, and Durham use the Anesthesia Care Team (ACT) model; rural and community hospitals frequently rely on independent CRNA practice.

Practice Model

  • Practice arrangement with physician required for most settings
  • ACT model: Charlotte (Atrium Health), Durham (Duke), Chapel Hill (UNC), Winston-Salem (Atrium/Wake Forest)
  • Independent CRNA model: rural Critical Access Hospitals and community surgical centers
  • North Carolina has NOT opted out of the federal Medicare physician supervision requirement — Medicare-certified facilities must have physician supervision unless a specific exception applies
  • Prescribing authority: CRNAs may prescribe controlled substances under DEA registration within their anesthesia practice scope

Application Requirements

  • Active North Carolina RN license (compact or NC-issued)
  • Current NBCRNA certification
  • Official nurse anesthesia program documentation
  • Completed ncbon.com online application + $75 fee
  • Criminal background check

Processing Timeline

NC Board of Nursing processes APRN applications in 6–10 weeks from complete submission. The Board issues a temporary license in some circumstances to allow practice while the permanent credential is finalized — contact the BON directly to request if a start date requires it.

What We See at Ava Health

North Carolina is a high-demand CRNA market with persistent rural shortages in the western mountains and eastern coastal plains. The Research Triangle and Charlotte metros offer competitive compensation within ACT models, while rural Critical Access Hospitals regularly post standalone CRNA roles with signing bonuses. North Carolina's growing population — particularly retirees in the western mountains and coastal communities — drives sustained demand for anesthesia services.
