How to Become a Registered Dental Hygienist in Missouri

Missouri's two major metro areas — Kansas City and St. Louis — anchor a healthy dental hygienist job market supported by large health systems, private DSOs, and a growing number of community health centers. The state's relatively low CE requirement and modest initial licensure fee make it a practical destination for candidates relocating from neighboring states. Here is a complete guide to the Missouri RDH licensing process.

Step 1: Complete an Accredited Dental Hygiene Program

Missouri requires graduation from a CODA-accredited dental hygiene program. In-state CODA-accredited programs include Missouri Southern State University (Joplin), State Fair Community College (Sedalia), and Missouri School of Dentistry & Oral Health (A.T. Still University, Kirksville). Both the Associate Degree in Dental Hygiene (ADDH, two years) and the Bachelor of Science in Dental Hygiene (BSDH) satisfy the Board's educational requirement.

The BSDH is preferred by larger health systems and academic employers, and online BSDH completion programs allow working hygienists to upgrade their credentials while employed.

Step 2: Pass the NBDHE

All Missouri applicants must pass the National Board Dental Hygiene Examination (NBDHE). The exam is 350 questions, covers all areas of dental hygiene science and clinical practice, and costs approximately $450. Schedule the exam through the JCNDE website. A passing score is required before your state application can be approved.

Step 3: Pass a Clinical Examination

Missouri accepts the following regional clinical examinations:

  • CRDTS (Central Regional Dental Testing Service) — Missouri is within the CRDTS service region, and test sites are readily accessible to in-state candidates.
  • ADEX (CDCA Dental Hygiene Examination) — also accepted for Missouri licensure.

Both assess live-patient periodontal debridement, dental hygiene radiographs, and overall clinical competency. Confirm testing dates and locations with each agency before scheduling.

Step 4: Apply for Your State License

Applications are submitted to the Missouri Dental Board. Required materials include:

  • Completed application and fee (~$50–$75 for initial licensure)
  • Official CODA-program transcripts
  • NBDHE passing score
  • Clinical exam passing score
  • Background check

Note: Missouri requires a separate Nitrous Oxide permit for dental hygienists who wish to administer nitrous oxide analgesia. This requires completion of an approved course and submission of a separate permit application to the Board. Many employers require or strongly prefer the nitrous oxide permit, so plan accordingly.

Missouri RDH licenses renew every two years.

Continuing Education Requirements

Missouri has one of the lowest CE requirements in the nation: 15 CE hours per two-year renewal cycle. There are no mandatory specific topic requirements at the state level (beyond general dental hygiene content), though most employers require current CPR/BLS certification. The Missouri Dental Hygienists' Association (MDHA) and ADA CERP-recognized online providers are common CE sources.

Missouri Dental Hygienist Salary Ranges

  • Entry-level (0–2 years): $52,000–$62,000/year
  • Mid-career (3–7 years): $62,000–$74,000/year
  • Experienced (8+ years): $74,000–$88,000/year

Kansas City and St. Louis metro hygienists earn at the top of Missouri's range. Rural Missouri positions run approximately 10–15% lower but often come with reduced overhead costs. Hygienists with nitrous oxide permits typically see a small but meaningful wage premium.
