- Ochsner Health (New Orleans / statewide): Largest private employer in LA; 90–120 days community/regional credentialing. Ochsner's Regional Health Collaborative extends to rural LA.
- LSU Health New Orleans (LSUHNO): 120–150 days academic. Public academic system spanning 9 LSU hospitals statewide.
- Tulane Medical Center (New Orleans): 120–150 days academic. HCA-operated with Tulane University academic affiliation — dual-track credentialing.
- Our Lady of the Lake (Baton Rouge): 90–120 days Catholic regional; largest hospital in Baton Rouge metro.
- Willis-Knighton Health (Shreveport): 90–120 days regional, dominant in NW Louisiana.
- University Health (Shreveport): 120–150 days academic; LSU Health Shreveport affiliation.
