Kentucky CRNA License 2026: APRN Requirements, Practice Model, Salary
Kentucky CRNAs are licensed as APRNs by the Kentucky Board of Nursing. Kentucky is a reduced practice state — a written collaborative agreement with a physician is required for CRNA practice. Kentucky has NOT opted out of the federal Medicare CRNA supervision requirement. Louisville and Lexington anchor the major market; Eastern Kentucky (Appalachian region) has persistent CRNA shortages driven by healthcare access challenges and a lower physician supply that makes CRNA-only anesthesia the primary model at Critical Access Hospitals.
Practice Model
- Written collaborative agreement with physician required
- Kentucky has NOT opted out of federal Medicare CRNA supervision requirement
- ACT model: UK HealthCare (Lexington), Norton Healthcare (Louisville), Baptist Health Louisville
- CRNA-independent (with collaborative agreement): Appalachian Regional Healthcare (ARH) system, rural CAH facilities across Eastern KY
- Eastern Kentucky's physician shortage means CRNAs often provide the sole anesthesia coverage for entire hospital service areas
Application Requirements
- Active Kentucky RN license (compact or KY-issued)
- Current NBCRNA certification
- Official nurse anesthesia program documentation
- Completed kbn.ky.gov online application + $100 fee
- Criminal background check via KSP
Processing Timeline
Kentucky Board of Nursing processes APRN applications in 6–10 weeks from complete submission. Temporary authorization to practice is not routinely issued. Kentucky's licensing process is standard for the region; complete applications rarely encounter extended delays.

What We See at Ava Health
Kentucky's Eastern Appalachian corridor is one of the most underserved CRNA markets in the Southeast. ARH's 12-hospital system and independent CAHs in Pike County, Harlan, and Whitesburg face persistent CRNA vacancies — these positions consistently offer above-average compensation to attract candidates to rural settings. Louisville and Lexington are competitive markets with standard Midwest compensation and a cost of living among the lowest of any major metro, giving Kentucky CRNAs strong purchasing power relative to nominal salary.
