Iowa PA License 2026: Physician Assistant Requirements, OTP Authority, and Salary
Iowa adopted Optimal Team Practice (OTP) for physician assistants in 2019, removing the requirement for state-filed supervision agreements. Iowa PAs document their collaborative physician relationships at the employer level, but the Iowa Board of Medicine does not require filing or approval of those arrangements. The state's healthcare market is dominated by two large regional health systems — UnityPoint Health and MercyOne — and the University of Iowa Hospitals in Iowa City. Rural western and southern Iowa has significant NHSC-eligible shortage designations.
Practice Model: Optimal Team Practice (OTP)
Iowa PAs establish collaborative relationships with physicians through their employing organization under the OTP framework. No state-filed supervision agreement is required. PAs may practice independently in most settings once licensed.
- No state supervision filing: Institutional collaborative relationships suffice
- Full prescriptive authority: Schedule II–V with DEA registration
- Rural practice: Significant independent practice in rural Iowa communities
Application Requirements
- Active NCCPA certification
- Graduation from ARC-PA accredited program
- Iowa Board of Medicine application and ~$120 fee
- Background check
- DEA registration (if prescribing controlled substances)
Processing Timeline
The Iowa Board of Medicine typically processes complete PA applications in 4–6 weeks. Total time to practice-ready including DEA: approximately 6–8 weeks.

Top PA Employers in Iowa
UnityPoint Health (Des Moines, Waterloo, Quad Cities, and 30+ Iowa hospitals) is the state's largest employer and primary PA recruiter by volume. UnityPoint has a structured PA career track and operates extensively in rural Iowa. MercyOne (Des Moines, Dubuque, Waterloo, and affiliates) competes with UnityPoint in most major Iowa markets. University of Iowa Hospitals and Clinics (Iowa City) is the state's academic flagship, recruiting for subspecialty and research-affiliated PA roles. Avera Health operates in western Iowa (Sioux City corridor) in addition to its South Dakota base. Rural Iowa — particularly the northwest and southwest quadrants — has extensive NHSC-eligible designations.
What We See at Ava Health
Iowa's OTP model and efficient Board of Medicine processing make it a straightforward PA deployment state. Sign-on bonuses in Des Moines and Iowa City are modest relative to shortage states; rural Iowa positions offer meaningfully larger incentives and NHSC eligibility for candidates with loan burdens. UnityPoint's rural system reach means candidates who want small-town Iowa practice can find positions with the backing of a large health system.
