Indiana CRNA License 2026: APRN Requirements, Practice Model, Salary
Indiana CRNAs are licensed as APRNs by the Indiana Professional Licensing Agency. Indiana is a reduced practice state — a written collaborative agreement with a physician is required. Indiana has NOT opted out of the federal Medicare CRNA supervision requirement. Indianapolis is a high-volume CRNA market anchored by Indiana University Health (the state's largest health system) and a robust network of community hospitals. Indiana has one of the lowest APRN license fees in the country at $50.
Practice Model
- Collaborative agreement with physician required
- Indiana has NOT opted out of federal Medicare CRNA supervision requirement
- ACT model: Indiana University Health, Eskenazi Health (Indianapolis)
- CRNA-independent (with collaborative agreement): Franciscan Health, Ascension St. Vincent community sites, rural CAH facilities
- Northern Indiana (South Bend, Elkhart) has strong CRNA demand with both ACT and CRNA-independent models
Application Requirements
- Active Indiana RN license (compact or IN-issued)
- Current NBCRNA certification
- Official nurse anesthesia program documentation
- Completed Indiana PLA online application + $50 fee
- Criminal background check via Indiana State Police
Processing Timeline
Indiana Professional Licensing Agency processes APRN applications in 4–8 weeks from complete submission. Indiana's licensing portal is efficient and the $50 fee reflects a lean administrative process; applications with complete documentation are among the faster-processing Midwest states.

What We See at Ava Health
Indiana delivers strong CRNA purchasing power — salaries in the $180K–$218K range against housing costs 35–45% below coastal markets. Indianapolis is a Midwest CRNA hub where IU Health's dominance means most new CRNAs encounter the system as their first employer. Fort Wayne and Evansville are regional hubs with lower competition for CRNA positions than Indianapolis. Rural Indiana CAH positions (Bedford, Bluffton, Rensselaer) routinely offer signing bonuses for extended vacancies.
