Clinical scenario questions

"Walk us through your admission of a 78-year-old presenting with sepsis from a UTI, septic shock, lactate 4.5, MAP 60 after 2L bolus."

Strong answers: confirm cultures drawn before antibiotics; broad-spectrum empiric (cover Pseudomonas + E. coli — Zosyn or cefepime + vanc if catheter-associated); third pressor consideration (Levo first-line); ICU-level care; central line; foley if not already; check for source (CT abd/pelvis, ECHO if endocarditis suspicion); discuss code status with patient/family early; document goals of care. Surviving Sepsis Bundle adherence.

"Patient on telemetry develops new-onset Afib RVR. Vitals: BP 90/50, HR 145, complaining of chest pain. What do you do?"

Unstable Afib with hypotension — synchronized cardioversion if hemodynamically unstable. "What's your typical census? What's your admission cap?"

"What's the comp structure?"

"Sign-on, retention, loan repayment?"

"What's the partnership/equity pathway (if private group)?"

"What's the schedule — 7-on-7-off? M-F? Block?"

"Average shift length and average notes-per-shift?"

Red flag questions to listen for

  • "You'll need to be flexible with the cap as the hospital grows" → cap creeps to 7-9 admissions
  • "We don't track productivity — we're a salaried group" + low base = potential underperformer culture
  • "Our turnover has been higher than expected lately" → ask why
  • "You'll cover when others are out" → confirm jeopardy/vacation coverage policy
  • "We're transitioning to NP/PA-heavy model" → autonomy may shift in 12-24 months
