Remove my listing
Ava Health’s public provider directory indexes information from the National Plan and Provider Enumeration System (NPPES NPI Registry), a public database maintained by the Centers for Medicare & Medicaid Services (CMS). All information displayed is legally public and sourced exclusively from this federal registry. We do not display email addresses, personal phone numbers, or any data outside of NPPES.
That said — we respect provider preferences and will remove any individual listing promptly upon request. You do not need to provide a reason. Removal is permanent (listings will not reappear unless you ask us to re-list you).
How to request removal
Send an email to privacy@avahealth.co with:
- Your full name as it appears in the listing
- Your NPI number (10 digits — visible on the listing page)
- The URL of the listing, if convenient
We remove listings within 3 business days of receiving the request. Once removed, the URL returns a 404 and Google will deindex it on its next crawl (typically 1–2 weeks).
Correcting a listing
If the information shown is out of date (city, specialty, credential, etc.), the fastest fix is to update your record in the NPPES registry directly — our directory revalidates weekly and picks up changes automatically. If you need us to expedite a correction, email the same address with “Correction:” in the subject line.
For patients and referring providers
This directory is for professional reference only. It does not constitute medical advice and is not an endorsement or recommendation of any specific provider. Before selecting or referring to a provider, verify their credentials and standing with the relevant state licensing board.
What we never display
- Email addresses
- Personal mobile or home phone numbers
- Home addresses
- Patient reviews or ratings
- Malpractice history, disciplinary action, or sanctions
- Any data not in the public NPPES registry
